Record DOT Physicals

Follow these steps to record a DOT Physical in TestVault.

Step 1. Create a DOT Physical test record

  1. Click "Tests" from the main navigation.
  2. Click the "Actions" button and select "New Specimen."
  3. Enter the person's company.
  4. For "donors", click "create new" and then enter the first and last name for the driver.
    • Alternatively if the person is already in the system, you can type the first few letters of their name and it will auto-complete.
  5. Choose "Physical" for "type".
  6. This will take you to the first page of the DOT Physical form.

Step 2. Complete the DOT Physical test record

  1. There are tabs on the top left which will take you to each of the individual pages.
  2. Once all pages are completed you can click on "Reports" on the top right and download both the full long form as well as the certification card.
